What Should I Bring to My Appointment?
To help us provide the best possible care, please bring the following items when applicable:
- Photo Identification: Driver’s license or other government-issued photo ID.
- Insurance Cards: Medical and vision insurance information, if applicable.
- Current Eyeglasses: Including prescription sunglasses or reading glasses you regularly wear.
- Contact Lenses: Your current lenses, contact lens boxes, or previous prescription information.
- Medication Information: A current list of medications, including eye drops and supplements.
- UNT Identification: Student or staff ID, if applicable.
