Understanding Your Eye Care Benefits
At North Texas Eye Clinic, we strive to make quality eye care accessible and help our patients understand their insurance benefits.
We accept a variety of medical and vision insurance plans. However, each insurance company offers different plans with specific coverage, benefits, and limitations.
Our team is happy to help verify your benefits and provide information about potential out-of-pocket costs before your appointment.
Medical Insurance vs. Vision Insurance
Many patients are surprised to learn that medical insurance and vision insurance cover different types of eye care.
Medical Insurance
Medical insurance generally covers medically necessary evaluations, diagnostic testing, and treatment of eye diseases and conditions, such as:
- Diabetic eye disease
- Glaucoma
- Dry eye disease
- Macular degeneration
- Eye infections and injuries
- Cataract evaluations
Coverage is subject to your insurance plan’s medical necessity requirements, deductibles, copayments, coinsurance, and other limitations.
Vision Insurance
Vision insurance typically provides benefits for routine eye care and vision correction, including:
- Routine comprehensive vision exams
- Eyeglass prescription evaluations
- Prescription glasses and lenses
- Contact lenses or contact lens allowances
Contact lens fitting and evaluation fees may be separate from your routine eye examination and may not be fully covered by your vision plan.
Understanding Your Insurance Benefits
Every insurance plan is different, even when offered by the same insurance company.
Your coverage may depend on your specific plan, employer benefits, provider network, and the services performed during your visit.
Some plans require prior authorization or referrals, while others may have limitations on the frequency of examinations or coverage for certain diagnostic procedures.
We encourage patients to become familiar with their insurance benefits before their appointment.
While our staff will gladly assist with verifying your eligibility and available benefits, information provided by insurance companies is not a guarantee of payment.
Final coverage and reimbursement are determined by your insurance company when your claim is processed.
Referrals & HMO Insurance Plans
Some insurance plans, particularly Health Maintenance Organization (HMO) plans, require a referral from your primary care physician before you can receive specialty medical eye care.
If your insurance requires a referral, please contact your primary care physician and arrange for the appropriate authorization before your appointment.
Referrals and authorizations generally must be obtained according to your insurance company’s requirements and may not be approved retroactively.
Please let our staff know if your insurance requires a referral when scheduling your visit.
Refraction & Contact Lens Evaluation Fees
A refraction is the portion of your eye examination used to determine your eyeglass prescription.
Although refraction is an important part of vision care, many medical insurance plans, including Original Medicare, do not cover this service.
Similarly, contact lens fittings and evaluations involve additional measurements, professional services, and follow-up care that may not be included in a standard eye examination benefit.
When these services are not covered by insurance, additional fees may apply.
Your Financial Responsibility
We are committed to providing clear information about our fees and helping you understand your insurance benefits.
Patients are responsible for applicable copayments, deductibles, coinsurance, and services not covered by their insurance plans, consistent with applicable insurance agreements and regulations.
Any benefit estimates provided before your visit are based on information available at that time. Your final financial responsibility may change after your insurance company processes the claim.
Preparing for Your Appointment
To help make your visit as smooth as possible, please:
- Bring your current medical and vision insurance cards.
- Inform our staff of any recent changes to your insurance.
- Obtain any required referrals or authorizations before your appointment.
- Let us know if you have questions about your benefits or expected costs.
